About the Role

Join Western CT Home Care Inc as a Physical Therapist – Home Care in United States. Summary: Provides services that identify, prevent, remediate and rehabilitate acute or prolonged physical dysfunction or pain. Evaluates and analyzes patients and therapeutic application, physical and chemical agents, exercise, instruction to patient/family and other procedures.

Key Responsibilities

  • Performs physical therapy assessment including functional limitations, rooms, capabilities, mobility status, endurance status, ADL status.
  • Identifies patient and family needs for other home health services and refers for same when needed.
  • Provides services that are ordered by the physician as indicated in the plan of care.
  • Communicates with the physician(s) involved in the plan of care and other health care providers as appropriate.
