• Lead the assessment and development of appropriate plan of care 2.

• Involve client, family and staff in execution of Plan of care 3.

• Intervene as warranted in response to DISCUS assessment 4.

• Incorporate interdisciplinary assessments into Plan of care such as BDP, IEP and other medical disciplines 5.
