Key Responsibilities

The Transitions of Care Pharmacist manages interdisciplinary clinical services and ensures pharmaceutical care programs are integrated throughout the health system. They facilitate medication reconciliation during admission and discharge while collaborating with healthcare teams to improve patient outcomes.

Requirements

Candidates must hold a Bachelor’s degree in Pharmacy and a valid Kentucky Board of Pharmacy license. At least one year of related work experience is required, with PGY-1 residency completion preferred.
