About the Role

Join Homewood Health Centre as a Social Worker – Temporary Full-Time in United States. Key Responsibilities

  • Conduct a comprehensive assessment for the client relevant to biopsychosocial history, diagnosis and ability.
  • Develop and evaluate treatment plans in collaboration with the clinical team, community supports and familie.
  • Provide individual counselling in support of patient’s specific needs and goals * Ensure continuity of care.

Requirements

  • Master’s Degree in Social Work
  • Current registration with the Ontario College of Social Workers
  • Training and educated in CBT and/or DBT modalities, and Mindfulness
  • Experience working with individuals with complex mental health and addiction issues, and trauma

  • Full-Time position.
