Physician Family Med/Internal Medicine or IM-Peds - With Focus on Medicaid and CHP+ Members

About the Role

Join Colorado Permanente Medical Group as a Family Medicine, Internal Medicine, or IM-Peds physician focused on Medicaid and CHP+ patients. Work in a hybrid model with 70% in clinic and 30% virtual care.

Key Responsibilities

• Provide comprehensive care to Medicaid and CHP+ patients.
• Participate in after-hours and weekend rotations.
• Collaborate with multidisciplinary teams including behavioral health and social work.
• Engage in community partnerships to improve health outcomes.

Requirements

• Board certified or eligible in Family Medicine, Internal Medicine, or IM-Peds.
• Willingness to take inbound on-demand calls (up to 30% FTE).
• Ability to participate in weekend and after-hours rotations.
• Experience with underserved populations preferred.
