Key Responsibilities

• Provides assessment, evaluation, and management to patients with minor, acute, chronic, & episodic illness & injuries. Including performance of follow up exams if required.

• Provides case management services to assist clients and families with the arrangement for, or refer patients to needed services.

• Conducts telephone calls back to patients requiring follow

• up care or provide test results. Also provides health counseling and education to patients and their families about their healthcare and any follow

Requirements

• Graduate of a master's level nurse practitioner program OR graduate from an accredited physician assistant program. Required Experience Qualifications

• 3 years work experience in a clinical setting within a health care organization Preferred Skills and Abilities

• Demonstrate organizational skills that allow timely patient appointments and appropriate required follow up.
