About the Role
Join UPHAMS CORNER HEALTH CENTER as a Occupational Therapist/ Home Care Coordinator in United States.
Key Responsibilities
- & Responsibilities Conduct initial Occupational Therapy and Home Care evaluations and ongoing reassessments for PACE participants to promote independence and community living.
- Perform home evaluations and safety screens semi-annually to optimize ADLs and ensure a safe environment.
- Develop and implement individualized care plans in collaboration with the Interdisciplinary Team (IDT).
- Educate and supervise participants, caregivers, and staff to ensure adherence to rehabilitation principles.
Requirements
- Experience: 2–3 years working with frail elderly or chronically ill individuals preferred.
- Comfortable working with a multicultural population.
- Strong interest in geriatric care; flexibility, creativity, and organizational skills.
- Licenses & Certifications: Valid Massachusetts Occupational Therapy license.
