Join UPHAMS CORNER HEALTH CENTER as a Occupational Therapist/ Home Care Coordinator in United States.

Key Responsibilities

• & Responsibilities Conduct initial Occupational Therapy and Home Care evaluations and ongoing reassessments for PACE participants to promote independence and community living.

• Perform home evaluations and safety screens semi-annually to optimize ADLs and ensure a safe environment.

• Develop and implement individualized care plans in collaboration with the Interdisciplinary Team (IDT).

• Educate and supervise participants, caregivers, and staff to ensure adherence to rehabilitation principles.

Requirements

• Experience: 2–3 years working with frail elderly or chronically ill individuals preferred.

• Comfortable working with a multicultural population.

• Strong interest in geriatric care; flexibility, creativity, and organizational skills.

• Licenses & Certifications: Valid Massachusetts Occupational Therapy license.

Benefits & Compensation

• Salary: $43-$58 per hour
