Key Responsibilities
- Heritage Home Health Physical Therapists (PTs) work with patients, their families, and other healthcare professionals in the patient’s place of residence.
- In addition to performing visits and completing coordination of care, the PT is the point of contact for all disciplines involved with providing care to the patients.
- The PT follows agency, state, and federal policies and procedures.
- The PT will consult with the patient’s primary care physician and attend meetings as required.
