Key Responsibilities
Conduct comprehensive in-home health evaluations, including physical exams and medical history reviews, to bridge gaps in care. Build trusted relationships with patients and provide connections to appropriate care based on unique individual needs.
Requirements
Must possess an active, unrestricted medical license (MD or DO) in the coverage area. Ability to travel locally and work comfortably in diverse home environments containing pets or allergens is required.
