Job Description
Occupational Therapist (OT) – Home Health
[Lake County] [Flexible Schedule] [Meaningful Patient Relationships]
What You'll Do
- Perform occupational therapy evaluations
- Develop individualized treatment plans
- Improve patient independence and safety
- Assess ADLs and functional limitations
- Educate caregivers and patients
- Coordinate care with interdisciplinary teams
What You'll Need
- Active Florida OT License
- Home health experience preferred
- HCHB experience preferred
- Strong clinical reasoning skills

Key Responsibilities

Requires an active Florida Occupational Therapist license. Home health experience and HCHB experience are preferred.
