Key Responsibilities

Provide evaluative, consultative, and treatment interventions to home health patients in their residences. Collaborate with the interdisciplinary team to develop and manage patient care plans while maintaining accurate clinical documentation.

Requirements

Must hold a valid and unrestricted professional license in the state of practice and be a graduate of an ACOTE accredited school. Prior home health experience is preferred.
