Key Responsibilities

The Behavioral Health Care Coordinator collaborates with health professionals to help members improve self-management abilities and health outcomes in a culturally specific manner, working with individuals who have complex medical, psychosocial, or addiction issues. Responsibilities include actively coordinating care, administering mental health assessments and treatment plans, making community referrals, and documenting all activities using the Electronic Medical Records system.

Requirements

Candidates must possess a Master’s Degree in a related clinical field such as Counseling Psychology or Social Work, along with a minimum of two years of experience in behavioral health, care management, or social services. An active Oregon LCSW or LPC license is required, as is the ability to pass criminal background, DMV, and pre-employment drug tests.
