The Physical Therapist will evaluate patient conditions and develop personalized treatment plans to be delivered in a home health setting. They are also responsible for supervising assistants, maintaining accurate documentation, and educating patients and families on home exercise programs.
Candidates must hold an active Physical Therapist license in Ohio and possess a valid driver's license with auto insurance. Experience with OASIS and WellSky/Kinser software is strongly preferred, along with current TB test and CPR certification.
