Key Responsibilities
The Physical Therapist provides mobile outpatient rehabilitation services to older adults in their homes or independent living facilities. They are responsible for evaluating patients, developing individualized treatment plans, and coordinating care to improve patient mobility and independence.
Requirements
Candidates must hold a valid Physical Therapy license and possess either a Master's degree in Physical Therapy or a Bachelor's degree with five years of relevant experience. A valid driver's license and reliable transportation are required for travel to patient locations.
