Key Responsibilities

• should be a practicing Physical Therapist in the state of Ohio with advanced training, experience, and skills treating a wide variety of Pelvic Floor conditions in both male and female patients.

• Will Do: Performs physical therapy evaluation, establishes and implements treatment plan for patients referred by a provider and assigned by the manager, clinical coordinator or designee.

• provide quality care and maintains accurate patient records.

Requirements

• Qualifications: Education: Graduate of an accredited physical therapy program; Doctorate degree preferred.

• Licensure / Certification: Current license to practice Physical Therapy in the state of Ohio; Current Basic Life Support (BLS) for Health Care Provider certification required.

• Experience: One year of clinical experience in respective area preferred.

• Effective Communication Skills Ability to make decisions and utilize good judgment concerning patient evaluation and treatment.
