RN Case Manager

Key Responsibilities

The RN Case Manager coordinates and manages patient care from admission through discharge to ensure continuity and quality of service. Responsibilities include maintaining accurate EMR documentation, processing physician orders, and collaborating with interdisciplinary teams to support patient outcomes.

Requirements

Candidates must hold a current, unencumbered Tennessee or Compact State RN license and BLS/CPR certification. A minimum of two years of home health experience and one year of EMR experience are required.
