Care Manager ECM/CalAIM, Behavioral Health Services

Key Responsibilities

The Care Manager will lead ECM services for a caseload of adult or youth beneficiaries, promoting organizational mission and building internal and external relationships. Essential duties include providing wrap-around services, conducting assessments, building care plans, and coordinating referrals for social support, housing, and benefits.

Requirements

A minimum of a Bachelor's degree in a related field, or an Associate's degree plus two years of mental health experience, is required, with experience in case management for populations with mental illness, substance use disorders, or homelessness. Candidates must possess strong interpersonal skills, the ability to work independently, cultural humility, and be able to obtain CPR/First Aid certification within 30 days.
