Join Old Glory Home Health Care & Assistive Care as a Home Health Physical Therapist in Greater Richmond, Virginia.

Key Responsibilities

• Perform initial evaluations, reassessments, and OASIS documentation.

• Develop and manage individualized treatment plans.

• Provide skilled therapy interventions to improve mobility, strength, balance, and function.

• Supervise and collaborate with Physical Therapist Assistants (PTAs) as applicable.

Requirements

• Active Physical Therapist (PT) license in Virginia.

• Minimum 1 year of clinical experience (home health experience preferred).

• Strong knowledge of OASIS documentation and Medicare guidelines (preferred).

• Excellent communication and interpersonal skills.

Benefits & Compensation

• Salary: $112K - $130K
