Key Responsibilities

Conduct comprehensive in-home health evaluations, including physical exams and medical history reviews, for health plan members. Bridge gaps in care by providing personalized health assessments and diagnostic services in patients' homes.

Requirements

Requires an active, unrestricted license and board certification as a Nurse Practitioner or Physician Assistant. Must be able to obtain multi-state licenses and travel up to 100% of the time within the assigned coverage area.
