Key Responsibilities

  • and responsibilities in accordance with Stanford Medicine Partners’ policies and procedures, including its Service Standards and its Code of Conduct.
  • Coordinates care and access to SHC and SMP resources for patients needing behavioral health services to improve patient outcome and experience.
  • Provides coordination of care to behavioral health resources for patients who cannot be seen by SHC resources or associated therapy vendors due to insurance or access issues.
  • Reviews all unprocessed referrals to determine best resources for care coordination so patient is not left with an unresolved referral.

Benefits & Compensation
