Key Responsibilities
Perform comprehensive patient assessments and document findings to ensure a smooth transition from rehabilitation facilities to home. Communicate pertinent medical findings to attending staff and ensure all quality elements and diagnoses are accurately documented.
Requirements
Requires an active and unrestricted Nurse Practitioner license in Colorado and a Master's degree in Nursing. Candidates must be board-certified and possess the ability to travel between various care sites.
