About the Role
Optimal Care seeks a Hospice RN Case Manager to provide skilled nursing care and coordinate hospice services for terminally ill patients in home or facility settings. This role requires clinical expertise and compassionate care.
Key Responsibilities
- Complete initial and ongoing nursing assessments.
- Manage patient caseload including care planning and coordination.
- Assess home safety and support systems.
- Provide support at time of death and bereavement assessments.

Requirements
- Graduate of accredited nursing program.
