About the Role

Join Silverado Hospice South Houston as a Full Time Hospice RN Case Manager. You will provide compassionate hospice and palliative care to patients with neurodegenerative and life-limiting illnesses in the Houston area.

Key Responsibilities

• Develop individualized Plans of Care for assigned patients.
• Collaborate with interdisciplinary teams, patients, families, and physicians.
• Provide skilled hospice and palliative nursing care.
• Conduct comprehensive nursing assessments.
• Supervise LVNs and Home Health Aides.
• Perform field case management duties.

Requirements

• Registered Nurse with valid Texas license.
• Minimum 1 year RN experience; hospice experience preferred.
• Ability to lift up to 50 pounds.
• Valid driver’s license and reliable transportation.
• Must clear background check, physical, and drug screening.
