About the Role

Join Silverado Hospice LA as a Full Time RN Case Manager. Provide skilled hospice and palliative care to patients in the Los Angeles area.

Key Responsibilities

Develop detailed Plans of Care.
Collaborate with interdisciplinary teams, patients, families, and physicians.
Provide hands-on hospice nursing care.
Conduct nursing evaluations and assessments.
Supervise LVNs and Home Health Aides.
Perform field case management.

Requirements

Registered Nurse with valid California license.
Minimum 1 year RN experience; hospice experience preferred.
Ability to lift up to 50 pounds.
Valid driver’s license and reliable transportation.
Must clear background check, physical, and drug screening.
