About the Role

Join Silverado Hospice LA as a Full Time RN Case Manager. Provide skilled hospice and palliative care to patients in the Los Angeles area.

Key Responsibilities

• Develop detailed Plans of Care.
• Collaborate with interdisciplinary teams, patients, families, and physicians.
• Provide hands-on hospice nursing care.
• Conduct nursing evaluations and assessments.
• Supervise LVNs and Home Health Aides.
• Perform field case management.

Requirements

• Registered Nurse with valid California license.
• Minimum 1 year RN experience; hospice experience preferred.
• Ability to lift up to 50 pounds.
• Valid driver’s license and reliable transportation.
• Must clear background check, physical, and drug screening.
