About the Role

Join Community Care, Inc. as a Behavioral Health Specialist – No Weekends in United States.

Key Responsibilities

  • Teams Resource - Consultation Conducts comprehensive Behavioral Health assessments, makes recommendations, provides technical assistance and assists the IDT staff in monitoring member outcomes.
  • Completes member assessments, and recommends positive supports and interventions.
  • Educates and trains IDT staff members in care provision for complex members.
  • Technical assistance is also provided to IDT staff when restrictive measures, waiver applications and/or other rights limitations/denials are part of a member’s plan.

Requirements

  • Education: Master's degree in Social Work or Counseling required Experience: REQUIRED: A minimum of 3 years of experience in providing behavioral health services.
  • For example, provision of BH case management or BH service coordination; or provision of individual psychotherapy/counseling.
  • Experience in development of behavior support plans for older adults and adults living with physical and/or developmental disabilities preferred.

  • Full-Time position.
  • Hybrid position - combination of remote and on-site.
