About the Role

Optimal Care is hiring a Hospice RN Case Manager to provide skilled nursing care and coordinate hospice services for terminally ill patients in home or facility settings. This role requires strong clinical skills and compassionate care.

Key Responsibilities

Conduct initial and ongoing nursing assessments.
Manage patient caseload including care planning and coordination.
