Job description

What You Will Do

The Behavioral Health Therapist II is a fully licensed, dually credentialed Masters level provider who provides clinical behavioral health services and coordination of care to patients, families and referral sources engaged with ProHealth Care. Integrated Behavioral Health Therapist Role:

  • Will facilitate urgent assessments and make recommendations for appropriate level of care
  • Referral sources will include our primary care physician partners, specialty clinic providers, and our own behavioral health providers.
  • Will also receive referrals for post-acute care follow-up visits from Emergency Department and medical floors.
  • Will provide bridge therapy sessions to patient’s within 7 days of discharge from an inpatient psychiatric and partial hospital program
  • Will offer short-term therapy (6 visits or less) if patient appropriate for that type of service
  • Will facilitate intake evaluations for PHC Partial Hospital Program
  • Will have understanding of clinical criteria for all levels of care, and be able to document recommendations using said clinical criteria
  • Develops and maintains positive relationships with providers with referral sources
  • Is able to identify and refer to community resources if patient’s needs exceed what we are able to offer here at ProHealth Care
  • Will utilize 90971, 90839, 90832, and 90834 billing codes

Requirements

Candidates must hold a Master's degree in a mental health field and possess active Wisconsin licensure as an LCSW, LPC, or LMFT. Additionally, they must hold a substance abuse counselor certification (SAC or CSAC) or a Substance Abuse Specialty Authorization.
