Develop and implement individualized, patient-centered plans of care for complex diagnoses. Collaborate with interdisciplinary teams to optimize patient outcomes and support safe discharge planning.

Requirements

Requires a Bachelor's degree and a valid Illinois Physical Therapy license. BLS certification and prior inpatient experience are also required.
