- · Facilitate patient engagement in treatment and education within a therapeutic environment.
- · Conduct individual, group, and family therapy utilizing evidence-based practices (CBT, MI, DBT).
- · Perform functions related to utilization review and discharge planning to promote safe and effective patient care transitions.
- · Communicate and collaborate with all members of the healthcare team to facilitate the episode of care.
Requirements
- 3 years of relevant healthcare, mental health, or community
- based experience. · Licensure: License
- eligible in Indiana as an LCSW, LMHC, LMFT, or LCAC. The candidate must have a minimum of a permanent LSW, LMHC
