• · Communicate and collaborate with all members of the healthcare team to facilitate the episode of care.

Requirements

• 3 years of relevant healthcare, mental health, or community

• based experience. · Licensure: License

• eligible in Indiana as an LCSW, LMHC, LMFT, or LCAC. The candidate must have a minimum of a permanent LSW, LMHC
