The RN Case Manager will manage an assigned patient caseload by performing skilled nursing visits, assessments, and coordinating individualized plans of care. They are responsible for maintaining accurate clinical documentation, including OASIS, and providing education to patients and caregivers regarding disease management and safety.
Candidates must hold an active RN license and possess recent experience in home health case management. A strong understanding of Medicare home health regulations, OASIS documentation, and the ability to work independently in a field-based setting are required.
