Maternal Health Case Manager, RN -Remote in WA

Key Responsibilities

The RN Case Manager will engage with Medicaid members to conduct comprehensive needs assessments and develop person-centered care plans. They will collaborate with internal teams and community resources to provide education, coaching, and support for members' medical and socioeconomic needs.

Requirements

Candidates must hold a current, unrestricted RN license in Washington and possess at least 2 years of maternal clinical experience. Proficiency in MS Office and the ability to maintain a secure, high-speed home office environment are also required.
