Key Responsibilities

As an Adult Case Manager in our Community Support Services (CSS) Program, you’ll be the bridge between individuals and the support systems they need to thrive.

You’ll help adults navigate mental health services, achieve goals outlined in their Individual Recovery Plans (IRPs), and foster connections to natural supports and community resources.
