They will collaborate with interdisciplinary teams to manage patient care plans, perform face-to-face eligibility encounters, and facilitate goals-of-care discussions.
Candidates must hold a Master of Science in Nursing (MSN) or higher and possess a current RN and Nurse Practitioner license. At least two years of experience in hospice, palliative care, or a related clinical field is preferred, along with national NP certification.
