The Discharge Specialist provides case management services and formulates discharge plans for patients transitioning to the next level of care. They also facilitate discharge planning groups and maintain patient records within the ACT database.
Candidates must have at least a high school diploma or GED and a minimum of one year of relevant healthcare experience. Proficiency in Microsoft Office or Google Suite and strong communication skills are required.
