Key Responsibilities

• Review provider discharge orders for accuracy and completeness and clarify discrepancies as needed.

• Provide comprehensive patient and family education on medications, treatments, follow up appointments, warning signs and self-care instructions.

• Instruct patients and families on topics such as health education, disease prevention, and answer patient questions to make informed decisions about their health and treatment.

• Use tools such as teach-back to confirm understanding.

Requirements

• BLS, ACLS, PALS certifications must be obtained through the AHA Full Time
