Key Responsibilities

The Social Worker provides essential emotional, social, and practical support to residents and their families in a long-term care setting, focusing on promoting well-being, dignity, and independence. Key duties include managing pre-admission procedures, acting as a resident/family advocate, coordinating care conferences, and leading social work programs to meet psychosocial needs.

Requirements

Candidates must be registered under the Social Work & Social Service Act, 1998, and hold a Bachelor of Social Work degree from a recognized school. A minimum of three years of clinical experience as a Social Worker, particularly with Geriatrics and familiarity with multi-disciplinary teams, is required.
