The RN Case Manager will facilitate interdisciplinary plans of care to guide patient progress toward clinical and quality outcomes. They are responsible for organizing patient care, delivering nursing services, and collaborating with the healthcare team to improve patient experience and quality of care.
Candidates must hold a current Registered Nurse license in Washington State and a BLS certification. A Bachelor's degree in Nursing is required, though 15+ years of experience may be considered in lieu of the degree.
