Key Responsibilities

Coordinate resources and information pertinent to client needs.

Coordinate referrals for client specific discharge planning; ensuring that the family actively participates in.

Requirements

Bachelor's degree

At least 1 year of experience working with children/adolescents

Valid Driver's License and clean driving record

Able to work Monday
