The Social Worker will provide acute care services including discharge planning, care coordination, resource connections, and long-term care planning for patients. Responsibilities also involve assessing complex biopsychosocial needs and providing interventions to support individualized treatment plans while acting as consultants within the system.
Candidates must possess a Bachelor’s degree in Social Work or a related field, along with a current Wisconsin Social Worker license. One year of work experience in social services within a hospital, medical clinic, county social service agency, or nursing home is desired.
