The Social Worker collaborates with the interdisciplinary team to perform psychosocial assessments, coordinate discharge planning, and provide crisis intervention for patients. They act as a liaison to community resources and advocate for patient needs to ensure safe and effective transitions of care.
Candidates must hold a Certified Social Worker (CSW) license in Nebraska and a minimum of a bachelor's degree in Social Work. Additionally, a Basic Life Support (BLS) certification from the American Heart Association is required upon hire.
