The Medical Social Worker evaluates and develops personalized care plans to address the emotional and social needs of home health patients. They also coordinate with the interdisciplinary team and supervise Social Work Assistants to ensure compliance with professional and governmental standards.
Candidates must have at least one year of experience in a health care, home health, or hospice setting and possess a valid driver's license with reliable transportation. A current social worker license and CPR certification are also required for this role.
