Provides psychosocial evaluations, counseling, and bereavement services to terminally ill patients and their families. Develops and implements social work plans of care in collaboration with an interdisciplinary hospice team.
Requires a Master's Degree in Social Work and a valid state Social Worker license. Candidates must possess a valid driver's license and have the ability to travel to patient homes.
