Population Health Nurse Navigator

Key Responsibilities

The Nurse Navigator coordinates patient-centered care, education, and advocacy for patients in value-based and home-based care models. They manage complex patient populations, facilitate transitions across the healthcare continuum, and address barriers to care to improve health outcomes.

Requirements

A Bachelor of Science in Nursing (BSN) and current Ohio RN licensure are required for this position. Candidates should have at least three years of recent clinical experience, with preference given to those with experience in primary care, case management, or post-acute settings.
