The Nurse Navigator facilitates patient entry into the healthcare system and coordinates care across the continuum for medically complex adult patients. They develop and monitor comprehensive care plans while partnering with multidisciplinary teams to ensure effective symptom management and patient education.
Candidates must hold a Bachelor's degree in Nursing and a current Massachusetts Registered Nurse license. A minimum of 5 years of nursing experience and strong leadership and critical thinking skills are required.
