Key Responsibilities
Provide primary care services to frail participants of the PACE program across home care, nursing homes, and day centers. Work independently as a PCP while collaborating with an interdisciplinary team of health professionals.
Requirements
Requires a Massachusetts Nurse Practitioner license, BLS certification, and at least one year of relevant experience, preferably in community health. Certification in Adult or Family Medicine is considered.
