As a Social Worker with WesleyLife At Home, you will:
- Explain WesleyLife Hospice and Palliative services to patients and families.
- Visit patient homes (wherever they call home) to assess the psychosocial status, safety concerns, and bereavement risk factors of patients and families/caregivers.
- Communicate findings to patient care teams to update care plans.
- Carry out social evaluations and plan interventions.
- Counsel patients and families/caregivers regarding stress and other identified coping difficulties.
- Provide information and referral services for patients and families/caregivers regarding financial, practical, and environmental needs.
- Serve as liaison with patients and families/caregivers and care providers or community agencies.
- Participate in caregiver, placement, and discharge planning when needed, including discharge to a higher level of care.
- LISW preferred, LMSW highly encourage to apply.

Key Responsibilities
The Social Worker assesses the psychosocial status and safety of hospice and palliative care patients in their homes. They provide counseling, coordinate care plans with medical teams, and facilitate referrals for financial and practical needs.
Requirements
Candidates should be a Licensed Master Social Worker (LMSW), with a Licensed Independent Social Worker (LISW) certification preferred. Applicants must be able to pass a pre-hire health assessment and drug screen.
