The RN Case Manager conducts initial patient assessments, develops and revises individualized care plans, and provides direct nursing care. They also coordinate with families, administer prescribed treatments, and manage the discharge planning process to ensure continuity of care.
Candidates must hold a current state RN license and possess at least one year of recent acute care experience. A valid driver's license, auto insurance, and BLS/CPR certification are also required.
