About the Role

Optimal Care seeks a Hospice RN Case Manager to provide skilled nursing care and case management for terminally ill patients in home or facility settings. The RN leads interdisciplinary hospice teams to deliver compassionate end-of-life care.

Key Responsibilities

Complete initial and ongoing nursing evaluations.
Manage patient caseload including assessment, planning, and care coordination.
Assess home safety and support systems.
Provide support at time of death and bereavement assessments.
Communicate effectively with patients, families, physicians, and care teams.

Requirements

Graduate of accredited nursing program (Diploma, Associate, or Baccalaureate).
Current Registered Nurse license.
Minimum 1 year clinical nursing experience.
Ability to work in home or nursing facility environments.
