The Physical Therapist will assess, plan, implement, and evaluate physical therapy treatment programs for home health patients. They will facilitate patient progression toward optimal levels of self-care and functional independence.
Candidates must be a licensed Physical Therapist in Illinois or eligible for licensure and have a minimum of one year of clinical experience or comprehensive clinical affiliations. Proficiency in home-based therapy equipment and basic computer skills are also required.
