The Inpatient Case Manager coordinates patient care transitions by completing assessments, participating in interdisciplinary rounds, and facilitating post-acute care referrals. They work closely with the care team to ensure treatment plans align with safe and effective discharge strategies.
Candidates must be graduates of an accredited nursing program with at least two years of relevant experience and a valid Registered Nurse license in Virginia. A Bachelor of Science in Nursing is required within five years of hire.
